New Family Health Plan - Administrative Fee

As of Aug. 1, 2015, we began instituting an annual administrative fee.  For more information please feel free to contact Revanne at [email protected] .

Our Administrative Fee starting Aug. 1, 2016,  has been revised.  We realize that the changes in health care have affected us all.  The Affordable Care Act has imposed higher deductibles for everyone.   Our new fee will be as follows and statements will be mailed in August:.

 Annual Family Fee: $175 per family regardless of the number of children or ages. 

Carmel Valley Pediatrics accepts the following PPO plans:

  • Blue Cross                     
  • Blue Shield
  • Cigna
  • Aetna
  • United Health Care
  • HealthNet

 

You will need to call your Insurance company and ensure our participation in your network to determine your coverage. We also suggest that you familiarize yourselves with your insurance policy. Please remember newborns must be added to your insurance policy within the first 30 days after birth. It is extremely important that you contact your insurance carrier to ensure that your newborn has been added to your policy.

At this time, we are NOT accepting any new HMO patients.  Our panel with Children's Physician's Medical Group/Rady's Childrens Health is presently closed.  

It is your responsibility to keep your insurance and personal information current in our files. Please be sure to notify us of any changes in insurance, address or phone numbers. As a courtesy to our patients, we will also be happy to bill all out of network insurance carriers.

Co-pays must be paid at the time of service. This is a contractual agreement that you have with your insurance company. After your claim has been reconciled with your insurance, you will receive a billing statement from our office. The amount on this statement will reflect your balance, and that balance is due and payable within 30 days. Any balance remaining on your account for services not covered by your insurance company is your responsibility. Please take the time to review all correspondence from your insurance carrier. EOBs (explanation of benefits) will detail how your claim was paid to Carmel Valley Pediatrics. If you see any discrepancies, it is your responsibility to contact your insurance carrier.

We send statements monthly and payment is due upon receipt. A $25 service charge will be automatically added to your bill for every 30 days your payment is late.  A check that has been return with NSF will also be charge a $25 fee. 

Insurance News

As you may already know, all individual and family insurance plans are changing.  In short, health care is changing rapidly, and it is very confusing for all of us.  We all dread it.  Our office does not verify benefits, so please verify with your insurance carrier that we are still in network with your new policy.

Please be sure to bring your new insurance card to you first appointment at Carmel Valley Pediatrics.  We will need to update every patient’s registration and we will need front and back copies of your insurance card.
